Referrals

Residential
Referrals are contingent upon Montgomery County Core Service Agency's waiting list. Therefore, you must call Montgomery County Core Service Agency at: 240-777-1400, and request a referral for residential rehabilitation services. All residential referrals must go through the Core Service Agency.

Outpatient Mental Health Centers
Threshold Services has two Outpatient Mental Health Centers:

Silver Spring - 8818 Georgia Avenue, Silver Spring, MD 20902 / Phone: 301-563-7000

Rockville - 751 Twinbrook Parkway, 1st Floor, Rockville, MD 20851 / Phone: 301-838-4100

If you have publicly funded insurance (Medicare, Medical Assistance, etc.), you must contact the Montgomery County Access Team to get a referral: 240-777-1770. If you have private insurance (Blue Cross/Blue Shield, etc.) you can contact the Centers directly for a referral.

Please note: the Silver Spring Mental Health Center offers Bi-lingual services, Spanish and English.

Renaissance
Placement in Renaissance can take several weeks to months from the date of assessment. Assessments can be scheduled upon completing an application and referral form. To receive an application and referral form, please scroll down to the request form below, or call (301) 754-1102, ext. 13.

The Nicole Castro Outreach Program
A potential client can usually get started in the Outreach Program within one week to two weeks from the time of assessment. You will need to first contact Threshold Services to receive a referral packet at (301) 754-1102, ext. 13, or complete the referral request form below. Then you will need to Contact MHP for authorization for a Rehabilitation Assessment for PRP services: 1-800-888-1965.
