Referral Process

For referral information please contact the centers directly at the number listed above. Threshold Services accepts Montgomery County residents with Medical Assistance (MA or Medicaid), Medicare and Medicaid, or PAC. Uninsured patients are reviewed on a case by case basis. Threshold Services is not paneled with any private insurance so we will need to confirm that any private insurance allows out of network providers. Threshold Services may ask for proof of income to determine ability to pay deductibles and/or co-payments.

The OMHC Director may need to approve private pay or private insured referrals before an intake appointment is scheduled.

The referral process occurs in two steps. Your first meeting is with a therapist. This session takes about 1.5 hours and you have some paperwork to complete. The second session is with a psychiatrist. This session takes about 30 minutes. After these sessions you will be assigned to a psychiatrist and/or therapist for ongoing treatment that you request or was recommended during the intake process.


Client Responsibilities

  1. You are responsible for any cost of treatment or co-payment should your insurance company or other third party fail to fully reimburse Threshold Services for services provided to you. Your copay:
    • Uninsured $2.00 per visit
    • Medicaid/MA $0 co-pay
    • PAC $0 co pay
    • Medicare only 45% per visit ($30.86/$42.81 therapy; $15.42 group; $27.32 med management; see staff re: other services)
    • Medicare/QMB $0
    • Medicare/Medicaid $0 co-pay
    • Private Insurance/Private Pay (Initial Evaluation: $181.12; Initial Psychiatric Evaluation: $181.12; Individual Therapy: $53.22/96.70; Group Therapy: $41.20; Medication Management: $72.68; Treatment Planning: $88.12)
  2. You are expected to attend all scheduled appointments. You are expected to be on time for all appointments. If you need to cancel or reschedule an appointment, you MUST call at least 24 hours in advance.
  3. You are expected to meet with a therapist at least every six months to complete an individual treatment plan. This is mandated by the state of Maryland for all clients. In addition, if you have Medical Assistance, PAC, or are uninsured you must meet with a therapist every six months to complete a brief survey so that we can get authorizations to keep providing you services. This is also a state mandate.
  4. If you do not provide 24 hour notice prior to a cancellation, it will be documented as a NO SHOW.
    • If you have a standing appointment, the space may no longer be held for you on a regular basis.
    • After your second NO SHOW, when you call to reschedule, you will have to see whoever is available to see you. This means that it may not be your therapist/psychiatrist.
    • If you do not show up for a scheduled appointment, it is your responsibility to call to reschedule. A staff member may attempt to reach you by phone at their discretion.
    • If you miss repeated scheduled appointments, a staff member may mail you a notice regarding the absences as well as copies of this notice to any of your other healthcare providers for whom we have a signed release of information on file.
    • After missing two (2) consecutive appointments, it is your responsibility to contact Threshold Services within five (5) business days to let us know if you wish to continue receiving services at our clinic.
    • Lack of participation in treatment is grounds for discharge from the program.
  5. If you need to change a standing appointment day or time, please discuss this with your therapist or psychiatrist. We will make every effort to provide an alternative appointment time that is convenient. Weekend and evening hours may be individually scheduled. Check with your provider for availability.
  6. You are responsible for making sure that you have the necessary prescriptions and refills of any medication until your next appointment. If you run out of medication before your next appointment, you may be asked to schedule an additional appointment in order to receive the prescription.
  7. You are responsible for informing Threshold Services of any changes in name, address, phone number, insurance, marital status, etc.


Professional Staff

Dennis Blair, LCSW-C
OHMC Director

Dennis has been working in mental health services for over 25 years. He has worked in community and hospital settings as well as a private practice. Dennis has worked with children, adolescents, and adults. He has experience with psychodrama and team building. He has been an administrator and supervisor for over 10 years.


Harvey Fernbach, MD, MPH
Psychiatrist

Dr. Fernbach has worked at Threshold Services since May 2005. He attended the Yale University School of Medicine 1966 – 1971 for both his MD and MPH degrees. He completed his general psychiatric residency at SUNY Downstate Medical Center from 1971 – 1974. Dr. Fernbach worked on clinical research at the National Institute of Mental Health 1974 – 1976. Has had a private practice since 1976. Dr. Fernbach is a board member of the Washington Psychiatric Society as well as the Co-director of the Washington DC chapter of Physicians for a National Health Program.


Rhona Bosin, LCSW-C
Therapist

Rhona Melody Bosin is a licensed clinical social worker. She earned her MSW from the University of Maryland at Baltimore and her BS from the University of Maryland, College Park. She has worked at Threshold Services for 4 years. She provides individual, family and group therapy, including dual diagnosis and art therapy groups. Rhona is part of Threshold Services’ new Family Psychoeducation Program that will be starting in the fall of 2007. She will co-facilitate multi family groups to improve communication and problem solving skills with clients and their families.


Cheryl D. Taylor, MPA, MSW, LGSW
Therapist

Cheryl D. Taylor received her master’s degree in social work from the University of Maryland at Baltimore in 2005 and joined Threshold Services in 2008. Ms. Taylor worked with a diverse array of clients at the Montgomery County Commission for Women and at a residential and day treatment program called Vesta, Inc. Her experience includes working with clients who have severe and persistent mental illness, dual diagnoses, depression, anxiety, bipolar, early abuse, relationship and communication issues and addictions.

Cheryl currently leads a weekly support group called Healthy Eating, Living and Body Image on Fridays from 11:30-12:45. In a relaxed setting, participants explore food addictions, eating disorders, impact of food on mood, making healthier food choices, mindful eating and improving the relationship to food. There are no diets or weigh-ins!


Kelly Gill, LCSW-C
Therapist

Kelly is currently a therapist with Threshold's Outpatient Mental Health Center. She received her undergraduate degree in Sociology/Psychology from the University of Maryland, College Park. She received her graduate degree in Clinical Social Work from the University of Pittsburgh. Kelly has worked as an Outreach North and South Counselor with Threshold Services and was the Program Director at their Bel Pre residential Community for almost two years.


Naomi C. Kabasela, LCPC, MA.
Therapist

I have a Master’s of Art degree from Bowie State University in Counseling Psychology. I have been with Threshold Services for 14 months. Previously, I have worked in RRP/PRP for 16 years and have been a psychotherapist for over eight years. At Threshold Services, I am responsible for doing clinical intakes, individual, group and family therapy. I currently facilitate two groups: Anger Management and Social Wellness. Shortly, I will also co-facilitate a Family Psychoeducation group.


Carolyn Levy, LCSW-C

Carolyn is a bilingual (English/Spanish) licensed clinical social worker. She obtained her Masters degree in Social Work from Columbia University, and her BA in Psychology from William & Mary. Carolyn studied Spanish in Guatemala and Madrid. She worked as a therapist for 14 years in inpatient and outpatient mental health facilities in New York and New Jersey, seeing children and adults in individual, family and group settings. Carolyn has experience with a wide range of mental health issues including substance abuse, trauma, psychosis, depression and anxiety. She joined the staff at Threshold in September of 2009 and she sees clients in both the Silver Spring and Rockville clinics. She leads a Spanish Support Group on Mondays in Silver Spring.
